| July 15, 2005 Softball walk-on tryout date announced In order to try out, you must meet the following requirements: - be enrolled as a full-time student at UTSA - complete a physical form and return it with the health insurance form, a copy of your insurance card (you must show proof of current health insurance) and emergency contact information by Aug. 26 - please pick up and return all medical forms to the Athletic Training Room located in the Convocation Center by Friday, Aug. 26 For more information about necessary medical forms and paperwork, please contact assistant athletic trainer Brenna Ellis at (210) 458-4178.
