Student-Athlete Medical Forms and Instructions
· Download and follow instructions for creating your ATS Medical Profile.
· You must have your medical insurance card with you in order to upload a picture of the front and back.
· Complete ALL the forms listed within the instructions.
NEW STUDENTS
· Bring documented proof of your Sickle Cell Trait Status.
· This test is mandatory for all student-athletes at UTSA.
- Student-athletes may either have a new blood test/screen done (sickle cell solubility test) with a QUEST DIAGNOSTICS LAB near you, using the lab request form

- Submit a Newborn Screen/blood test from the hospital where you were born.
